How often do firearm manufacturers perform a SFMEA (Systems FMEA) and a corresponding FMEM (Failure Mode Effects Management) on firearm designs, or is that not an industry practice in the firearms field?
I don't want to single out any firearm manufacturers or designs, but A LOT of failures could have been prevented by something as simple as a SMFEA and a FMEM. When designing other military equipment we have to do this, but I see glaring errors in firearm designs that would have been caught by a baby engineer doing a SMFEA. A lot of problems can be easily avoided.

The engineering parents in my neighborhood worked for Mellonics (Litton Industries) for Army contracts during the improvement program for the M16A1. It was cool to hear those engineers discuss what was needed to make the M16A1 better for the Army. They were very upset at the Marine Corps Officers and the incompetence of the Ordnance Corps and Natick when it came to the adoption of the M16A2. The engineers in the early 1980's well analyzed the ergonomic and reliability needs that ultimately became part of the ARs we know today.

Sorry for the long question. Why does it seem like the traditional American companies like S&W, Winchester, Remington, colt, Ruger, etc. are unable to compete in today’s market? They seem to be more tradition focused instead of adapting to the modern market. S&W might be the most successful with their M&P line being somewhat popular with civilians and police. They really can’t compete for contracts like HK, Glock, Sig, FN and the like can. I have no problem with foreign companies,It just seems like we struggle to produce quality gear for our own military. Feel free to correct me if I’m wrong.

Are striker-fired pistols inherently more reliable than hammer-fired? Hammer-fired has more parts that can break, but also can overcome light primer strikes with another pull of the trigger. Is it largely a wash, or can you say one is more reliable than the other?

How common are Calcium Carbonate and/or Titanium Dioxide propellant additives seen in GSR analysis for small arms? I am seeing higher concentrations of Calcium Carbonate and more commonly seeing Titanium Dioxide when analyzing insoluble deposits in AR rifles when single or even double base propellants are used. I wonder if this is caused by some manufacturers using decommissioned sources of artillery propellants in ammunition. Aqueous solvents and mechanical removal methods are needed, because CLP does not work.

Are you familiar with with the Dead Foot Arms Folding Stock? They cut the BCG 1 3/8 inches shorter and have a spring mechanism so it can fire while folded (the only one on an AR I know of). Do you see any reliability issues with this? Do you think you lose anything in accuracy?
