Pat's Place:

Our unique signature program that provides diabetes education in a vacation setting. Specifically targeted to newly diagnosed youth, technology updates, and burnout. This program provides the entire famiy an opportunity to apply diabetes education principles in a supportive and fun environment.

Diabetes Education

Diabetes Education Center with individual and group trainings Educational support for people living with all types of diabetes with a special focus towards teens/young adults living with type 1 diabetes.

Medication Therapy Management

We will complete a thorough review of your diabetes regimen and communicate with your diabetes provider to maximize your diabetes care.

Diabetes Technology Assessments and Trainings

Professional support for Continuous Glucose Monitors (CGMs) and insulin pump education and trainings.
