Care Management provides individualized support to older adults and caregivers confronted with the challenges of aging.
Our team acts as navigators and companions, helping people age with dignity and compassionate support. But not everyone can afford our service - we need your help to support our Equity Fund which provides a sliding scale fee to our lowest-income clients.
Each month, Equity Funds represent about 25% of the services we provide with much more needed.
