THE PROBLEM
Each year 3.6 million Americans miss or delay health care appointments because they lack reliable transportation. The lack of consistent and affordable transportation options disproportionality effects our low income and older adult populations, creating a barrier to health care for those most at risk of being vulnerable. Those without reliable transportation are often forced to forgo preventive care and only seek help in medical emergencies, increasing the cost of care and decreasing the likelihood of positive health outcomes.
THE SOLUTION
A human-centered mobility solutions, coordinating transportation for patients frequently missing health care and need based appointments. At Mobility Match, we take the time to get to know each patient and identify their needs as they relate to transportation inequity. By taking the time to get to the root of the problem, we are able to connect people to the most appropriate resources to ensure sustainable solutions.

In order to ensure our users have equitable transportation access to care, we need your support. The average trip cost will vary depending on the mode of transportation, but we are looking to raise $25,000 to fund our three month pilot program with 50 individuals. Your tax deductible donation will provide a valuable resource and cover the cost of trips for those frequently missing health care and need based appointments due to a lack of reliable transportation.
