Michigan Creativity - Summer Camp

There are three parts to registration for Michigan Creativity Camp:

1) Complete Payment and Registration Confirmation (this form - page 2 includes YOUR info as the person paying for camp)

2) Provide Camper information (you will automatically be directed there after payment is submitted)

3) Health & Insurance Information (will need to be emailed before camp - you will be contacted to request this information)

Please note: all registrations are final.  Cancellations will only be refunded 50% of their registration fee, as long as the cancellation is requested by June 1.  No refunds will be provided after June 1, 2020.
