Giving Tuesday Annual Campaign
Community Health & Wellness saw over 7,000 patients for more than 37,000 appointments this year. Our community health workers addressed the social determinants of health (SDOH), which are the non-medical factors that affect health outcomes and the conditions in which people are born, grow, work, live and age.
2025 was a difficult year.
We saw people lose benefits, unable to feed their families. CHWC stepped in, providing access to food at our three health centers and giving gift cards to local grocery stores. We saw patients who could not afford to pay their rent and feared being evicted. CHWC stepped in, providing a month's rent. We had individuals walk into one of our three health centers, needing health care but uninsured. Again, CHWC stepped in, providing our services regardless of their insurance status or ability to pay. No matter what someone's circumstances are, no one is ever turned away.
