50th Anniversary Gala

It is with the utmost pleasure that I invite you to our 50th Anniversary Gala, which will be held at The Second City’s 1959 Kitchen & Bar on November 15, 2022. 


The uncertainty, isolation, and loss the world experienced during these past two years has demonstrated how unequivocally important our mental, emotional, and social well-being are to living and leading healthy lives. With the psychological impact of the pandemic likely to linger for years, access to quality care is now more than ever a public health necessity, and must be considered as such – it can no longer remain a privilege of the insured. Join us this evening and help make hope and healing a reality for all. 


Seeking to reconnect with old friends and new supporters in a celebration of our impact, resilience, and growth throughout the most trying of the pandemic years; we want to come back to our central message that there is strength in community. Since 1972, C4 has served communities on Chicago’s west and north side as the leading provider of community-based and crisis-de-escalation mental health services. Annually, we seek to equip more than 7,000 children, adults, and families to help them move from trauma to living, to working and thriving within their community. The population that C4 serves is almost entirely clients with public insurance or no insurance — C4 is largely a Medicaid fee-for-service agency, and due to the low reimbursement rates of Medicaid, we experience thin margins in our operations.  


Tickets to this year’s event will be utilized to support the capital and operating needs of our programs, including support for the 988 Lifeline, for which C4 is Chicago’s exclusive provider.

Tickets - $150 per person, or $1500 for a table of 6; ticket price includes a cocktail reception featuring a premium bar and host of hors d'oeuvres {additional sponsorship options listed on the following page}
